I have a male beta fish with a full belly and string-like tissue hanging from his belly. The tissue string is about 2 inches long and has a tissue ball at the end of it. He only swims at the top of the tank, and barely moves. He is not dead. He moves his fins occasionally but not much.

are you sure he is alive? because the description sounds dead, if you have a net, good, if not get one, or something long, and just stir the water around him, if he moves, HE LIVES! Then i don't know about the sick part....

Are you sure that it's not just poop? My beta does this about once a month or so (at least that I notice). It will just fall off and your fish should be fine.
